After the interaction of hormones & receptors certain changes occurs in the functioning of cells of growth plate of long bones tissue so that growth plate cells starts growing in thickness & size & more & more new bone starts depositing in the bone thus growth plate size gradually starts increasing. Simultaneously it also increases the blood supply to growth plate tissue leading to more availability of growth factors to the growth plate tissue resulting in faster growth of growth plate tissue. These hormones & growth factors are in high concentration during peak of secondary sexual characters development i.e. puberty leading to persistent stimulation of growth plate for next two to three years i.e. up to completion of sexual development. Thus in children in two to three years full height growth occurs.
Causes of short height are:

The health care provider will perform a physical examination. The child’s height, weight, and arm and leg lengths will be measured. If the short stature appears to be a sign of some medical abnormality, further tests including laboratory studies and x-rays may be needed. To learn more about possible causes, the health care provider will ask questions, such as:

- How tall are the brothers or sisters?
- Are there other relatives that are less than average height?
- Have any family members been diagnosed with a disorder associated with short stature?
- At what age did the parents start puberty?
- What was the child’s birth like?
- How is the child’s diet?
- Has the child begun to show signs of puberty?
- At what age did puberty signs begin?
- Has the child always been on the small side of the growth charts?
- Was the child growing normally and then the rate of growth began to slow?
- What other symptoms are also present?
If the short stature appears to run in the family or is due to delayed growth, a bone age x-ray will be done. This test determines if the bone age is appropriate for the patient’s age. For instance, if an 8-year- old child is only as tall as a 6 year old, the bone age x-ray would suggest that growth was merely delayed and that future height should be normal. Girls with short stature should have a cerotype done to check for certain genetic diseases such as Turner syndrome.
Other tests may include:

Although your health care provider keeps records of height and weight from routine examinations, you may find it helpful to keep your own records. You may want to bring these records to your health care provider’s attention if the growth seems slow or the child seems small.

Detailed History: First we take detail history about diet, past illness & other relevant history to reach on probable cause for shortness of height. Birth history (breech delivery, preterm), Height, Weight at birth, Early developmental milestones, Scholastic performance, Dietary intake (in present & past)
Any systemic disease (in present & past), History suggestive of Hypothyroidism, Cushing’s disease or syndrome, Growth hormone deficiency, History of steroid intake Psycho-social history, Family history of Short height & delayed growth & puberty (in parents, 1st & 2nd degree relatives) Old height & weight records.
Physical Examination: We look for anemia, signs of malnutrition, Rickets, Neck for goiter, then we do Systemic examination- Organomegaly & CVS, Respiratory system, Central Nervous system is examined for any cause in CNS & Intellect, Fundus for Papilloedema &optic-atrophy. We also Look for signs of Growth hormone deficiency, Hypothyroidism, for congenital anomalies in face, neck, hand, chest, leg & body for primordial dwarf, turner syndrome, Measure Weight, calculate weight age, calculate relationship of weight age to height age.
If weight for height is less (weight. age less than height. age) i.e. child is underweight the chance of Nutritional short height or malabsorption as cause is more likely. If weight for height is more (i.e. weight. age > height. age) then chances of endocrine i.e. hormone disorder as cause for short height is more likely.
Investigation & Diagnosis
After detail history & examination whatever hormone deficiency is suspected is investigated.
For this we do following tests:
- Hormone test: as growth hormones, thyroid (Free T4, Free T3, TSH), FSH, LH, testosterone, estradiol, IGF-1, IGFBP-3 etc.
- Bio-chemical test: (Hb., ESR, GBP, Alkaline.Phosphatase, Calcium, Phosphorus, Urea, creatinine, Urine Protein, M/E ,Fasting urinary ph , Serum bicarbonate,Serum Potassium ,Serum protein, SGPT Stool Fat , Tests to diagnose other systemic diseases x-ray Skull, Bone age (skeletal age) test.
- Short height due to Growth hormone deficiency: Growth hormone deficiency is one of the common causes of short height. Growth hormone formation becomes less in body due to inability of growth hormone secreting glands to make GH hormone from birth due to defect in gland functioning itself which is called congenital growth hormone deficiency. Then other cause are those cases in which growth hormone deficiency occurs later in life due to some damage to growth hormone forming gland later in life such as due to idiopathic growth hormone deficiency , tumour or trauma. G.H. ineffectiveness, hypopituitarism, Isolated growth hormone deficiency, Biologically inactive growth hormone, acquired idiopathic growth hormone deficiency, psychosocial deprivation syndrome.

What is Growth?
A child’s physical growth refers to the increases in height and weight and other body changes that occur as a child matures. Hair grows; teeth come in, come out, and come in again; and eventually puberty hits. It’s all part of the growth process.
The first year of an infant’s life is a time of astonishing change. During this time, a baby will grow rapidly. On average, babies grow 10 inches (25 centimeters) in length while tripling their birth weights by their first birthday.
Given all the growth that occurs in the first year of life, new parents may be surprised when their child doesn’t continue to grow through the roof. But no child continues the rate of growth experienced during infancy. After age 1, a baby’s growth in length slows considerably, and by 2 years, growth in height usually continues at a fairly steady rate of approximately 2 1/2 inches (6 centimeters) per year until adolescence. No child grows at a perfectly steady rate throughout this period of childhood, however. Weeks or months of slightly slower growth alternate with mini “growth spurts” in normal children. Kids actually tend to grow a bit faster in the spring than during other times of the year! A major growth spurt occurs at the time of puberty. Usually kids enter puberty between age 8 to 13 years in girls and 10 to 15 years in boys. Puberty lasts about 2 to 5 years. This growth spurt is associated with sexual development, which includes the appearance of pubic and underarm hair, the growth and development of sex organs, and in girls, the onset of menstruation. By the time girls reach age 15 and boys reach age 16 or 17, the growth associated with puberty will have ended for most teens and they will have reached physical maturity.
At the Doctor’s Office
Beginning in infancy, a child will visit a doctor for regular checkups. At each of these visits, the doctor will record the child’s height and weight on a growth chart. This chart shows the doctor and parents how the child’s height and weight compare to that of other children the same age. This valuable tool can help your child’s doctor determine whether your child is growing at an appropriate rate or whether there might be any problems.
What Can You Do?
There are several things you can do to help ensure that your child grows and develops normally. Make sure he or she gets the following, which are critical to your child’s overall health and wellness:
enough rest – Sleep patterns vary by age and individual child, but most kids need an average of 10 to 12 hours of sleep per night. Sleep gives growing bodies the rest they need to continue growing properly.
proper nutrition – A balanced diet full of essential vitamins and minerals will help your child reach his or her full growth potential.
adequate exercise – Because obesity is a growing problem in kids, parents should make sure that their children exercise regularly, as well as receive proper nutrition. Bicycling, hiking, in-line skating, sports, or any enjoyable activity that will motivate your kid to get moving will promote good health and fitness and help your child maintain a healthy weight.
Talking to Your Child About Growth
Children differ in growth and development during childhood – just like adults, some kids are taller or shorter. Generally, girls hit puberty earlier than boys, although some girls might lag behind their peers in breast development and the onset of menstruation. All of this is usually completely normal.
Try to avoid comparing growth among siblings or other children. Drawing attention to a child’s height, for example, will only make that child feel self-conscious about his or her size. Encourage your child to accept his or her own growth and development. Explain that some children grow and develop at different rates – and late bloomers generally will catch up eventually. Children have many questions about growth, from why their teeth fall out to difficult or potentially embarrassing topics like breast development or perspiration. Answer questions honestly and even initiate conversations about growth to help your child understand the many changes he or she is facing. This will help your child accept the changes positively. If you’re uncomfortable discussing these topics or avoid the subjects altogether, your child may interpret that to mean there’s something shameful about the changes he or she is going through and may be less likely to talk to you about any concerns.
Kids who are short often face teasing by peers and may need help coping. You can help a short child by supporting his or her self-esteem. For example, although it might be difficult for a small boy to make the football team, emphasizing alternatives, such as soccer or tennis, may make him feel better about himself and what he can do. A small teen might have concerns about dating, driving, and participating in sports. It’s important to try to understand your child’s feelings and to keep the lines of communication open. Another thing you can do to boost your child’s mood is to encourage activities that don’t focus on height or weight. Emphasize special talents or individual qualities, such as musical aptitude or a love of literature.
If You Suspect a Problem
Parents sometimes become concerned about their child’s growth and development, but it can be reassuring to know that most children who are short or delayed in development are healthy and normal. For example, shorter parents tend to have shorter children and not all kids develop at the same rate.
If you’re concerned about your child’s growth, the first step is to consult your child’s doctor, who can thoroughly evaluate your child and your family history and, if necessary, order some basic lab tests to see if there may be a medical condition affecting growth. The doctor may choose to monitor your child’s growth more frequently on a growth chart.
Bones grow in length at the epiphyseal plate by a process that is similar to endochondral ossification. The cartilage in the region of the epiphyseal plate next to the epiphysis continues to grow by mitosis. The chondrocytes, in the region next to the diaphysis, age and degenerate. Osteoblasts move in and ossify the matrix to form bone. This process continues throughout childhood and the adolescent years until the cartilage growth slows and finally stops. When cartilage growth ceases, usually in the early twenties, the epiphyseal plate completely ossifies so that only a thin epiphyseal line remains and the bones can no longer grow in length. Bone growth is under the influence of growth hormone from the anterior pituitary gland and sex hormones from the ovaries and testes.
Even though bones stop growing in length in early adulthood, they can continue to increase in thickness or diameter throughout life in response to stress from increased muscle activity or to weight. The increase in diameter is called appositional growth. Osteoblasts in the periosteum form compact bone around the external bone surface. At the same time, osteoclasts in the endosteum break down bone on the internal bone surface, around the medullary cavity. These two processes together increase the diameter of the bone and, at the same time, keep the bone from becoming excessively heavy and bulky.

Human Growth Hormone (HGH) is the most abundant hormone produced by the pituitary gland (pituitary is one of the endocrine glands). The pituitary gland is located in the center of the brain. HGH is also a very complex hormone. It is made up of 191 amino acids making it fairly large for a hormone. In fact, it is the largest protein created by the Pituitary gland. HGH secretion reaches its peak in the body during adolescence. This makes sense because HGH helps stimulate our body to grow. But, HGH secretion does not stop after adolescence. Our body continue to produce HGH usually in short bursts during deep sleep. Growth Hormone is known to be critical for tissue repair, muscle growth, healing, brain function, physical and mental health, bone strength, energy and metabolism. In short, it is very important to just about every aspect of our life!
What is Recombinant Growth Hormone (GH)?
Recombinant Growth Hormone is GH that is synthesized in the lab. It is a biosynthetic hormone that is identical to human growth hormone, but it is synthesized in the lab. Creating an exact replicate of HGH was not an easy task.
What is IGF-1?
IGF-1 stands for Insulin-like Growth Factor 1. IGF-1 is also known as Somatomedin-C. As important as HGH is, it does not last long in our bloodstream. In just a few short minutes our liver absorbs HGH and converts it into growth factors. IGF-1 is the most important growth factor that is produced. So, IGF-1 is a hormone just like HGH, but it is easier to measure in the body because it stays in our bloodstream longer than HGH. You can think of HGH as the hormone that gets the ball rolling, but IGF-1 does most of the work.
What is a Secretagogue?
A secretagogue (se-krët´ah-gog) is an agent that stimulates secretion. For now that is all you need to know about these agents with a funny name, but as we move on the functions of secretagogues and HGH enhancement will become important.
Physiologic Effects of Growth Hormone
A critical concept in understanding growth hormone activity is that it has two distinct types of effects:
Direct effects are the result of growth hormone binding its receptor on target cells. Fat cells (adipocytes), for example, have growth hormone receptors, and growth hormone stimulates them to break down triglyceride and suppresses their ability to take up and accumulate circulating lipids.
Indirect effects are mediated primarily by a insulin-like growth factor-I (IGF-I), a hormone that is secreted from the liver and other tissues in response to growth hormone. A majority of the growth promoting effects of growth hormone is actually due to IGF-I acting on its target cells.
Effects on Growth
Growth is a very complex process, and requires the coordinated action of several hormones. The major role of growth hormone in stimulating body growth is to stimulate the liver and other tissues to secrete IGF-I. IGF-I stimulates proliferation of chondrocytes (cartilage cells), resulting in bone growth. Growth hormone does seem to have a direct effect on bone growth in stimulating differentiation of chondrocytes. IGF-I also appears to be the key player in muscle growth. It stimulates both the differentiation and proliferation of myoblasts. It also stimulates amino acid uptake and protein synthesis in muscle and other tissues.
Control of Growth Hormone Secretion
Production of growth hormone is modulated by many factors, including stress, exercise, nutrition, sleep and growth hormone itself. However, its primary controllers are two hypothalamic hormones and one hormone from the stomach:
Growth hormone-releasing hormone (GHRH) is a hypothalamic peptide that stimulates both the synthesis and secretion of growth hormone.
Somatostatin (SS) is a peptide produced by several tissues in the body, including the hypothalamus. Somatostatin inhibits growth hormone release in response to GHRH and to other stimulatory factors such as low blood glucose concentration.
Ghrelin is a peptide hormone secreted from the stomach. Ghrelin binds to receptors on somatotrophs and potently stimulates secretion of growth hormone.
Growth hormone secretion is also part of a negative feedback loop involving IGF-I. High blood levels of IGF-I lead to decreased secretion of growth hormone not only by directly suppressing the somatotroph, but by stimulating release of somatostatin from the hypothalamus.
Growth hormone also feeds back to inhibit GHRH secretion and probably has a direct (autocrine) inhibitory effect on secretion from the somatotroph.
Integration of all the factors that affect growth hormone synthesis and secretion lead to a pulsatile pattern of release. Basal concentrations of growth hormone in blood are very low. In children and young adults, the most intense period of growth hormone release is shortly after the onset of deep sleep.
Growth hormone deficiency
Growth Hormone Deficiency (GHD) is a medical condition in which the body does not produce enough growth hormone (GH). Growth hormone, also called somatotropin, is a polypeptide hormone which stimulates growth and cell reproduction. HGH also refers to human growth hormone but this older abbreviation has begun to develop paradoxical connotations, particularly in relation to the peddling of medically unnecessary GH supplementation (so-called HGH quackery).
Growth hormone deficiency has different effects at different ages. In newborn infants the primary manifestations may be hypoglycemia or micropenis, while in later infancy and childhood, growth failure is more likely. Deficiency in adults is rare, but may feature diminished lean body mass, poor bone density, and a number of physical and psychological symptoms. Psychological symptoms include poor memory, social withdrawal, and depression, while physical symptoms may include loss of strength, stamina, and musculature. Other hormonal or glandular disorders frequently coincide with diminished growth hormone production.
GH deficiency can be congenital or acquired in childhood or adult life. It can be partial or complete. It is usually permanent, but sometimes transient. It may be an isolated deficiency or occur in association with deficiencies of other pituitary hormones.
The term hypopituitarism is often used interchangeably with GH deficiency by endocrinologists but more often denotes GH deficiency plus deficiency of at least one other anterior pituitary hormone. When GH deficiency (usually with other anterior pituitary deficiencies) is associated with posterior pituitary hormone deficiency (usually diabetes insipidus) the condition is termed panhypopituitarism.
Signs and symptoms
The incidence of idiopathic GHD in infants is about 1 in every 3800 live births, and rates in older children are rising as more children survive childhood cancers which are treated with radiotherapy, although exact rates are hard to obtain. Severe prenatal deficiency of GH, as occurs in congenital hypopituitarism, has little effect on fetal growth. However, prenatal and congenital deficiency can reduce the size of a male’s penis, especially when gonadotropins are also deficient. Besides micropenis, additional consequences of severe deficiency in the first days of life can include hypoglycemia and exaggerated jaundice (both direct and indirect hyperbilirubinemia). Female infants will lack the microphallus of course but may suffer from hypoglycemia and jaundice.
Even congenital GH deficiency does not usually impair length growth until after the first few months of life. From late in the first year until mid teens, poor growth and/or shortness is the hallmark of childhood GH deficiency. Growth is not as severely affected in GH deficiency as in untreated hypothyroidism, but growth at about half the usual velocity for age is typical. It tends to be accompanied by delayed physical maturation so that bone maturation and puberty may be several years delayed. When severe GH deficiency is present from birth and never treated, adult heights can be as short as 48-58 inches (122-147 cm).
Severe GH deficiency in early childhood also results in slower muscular development, so that gross motor milestones such as standing, walking, and jumping may be delayed. Body composition (i.e., the relative amounts of bone, muscle, and fat) is affected in many children with severe deficiency, so that mild to moderate chubbiness is common (though GH deficiency alone rarely causes severe obesity). Some severely GH-deficient children have recognizable, cherubic facial features characterized by maxillary hypoplasia and forehead prominence (said to resemble a kewpie doll).
Children have sparse hair growth and frontal recession, and pili torti and trichorrhexis nodosa are also sometimes present.
The incidence of genuine adult-onset GHD, normally due to pituitary tumours, is estimated at 10 per million.
Recognised effects include:
· Increased 5-alpha-reductase
· Reduced sex hormone binding globulin (SHBG)
· Reduced muscle mass and strength
· Baldness in men
· Reduced bone mass and osteoporosis
· Reduced quality of life, particularly energy levels
· Impaired concentration and loss of memory
· Increased body fat, particularly around the waistline
· Lipid abnormalities, particularly raised LDL cholesterol
· Insulin resistance
· Increased levels of fibrinogen and plasminogen activator inhibitor
· Cardiac dysfunction, including a thickened intima media
Childhood GHD commonly has no identifiable cause (idiopathic), and adult-onset GHD is commonly due to pituitary tumours and their treatment or to cranial irradiation. A fuller list of causes includes:
· mutations of specific genes (e.g., GHRHR, GH1)
· congenital diseases such as Prader-Willi syndrome, Turner syndrome, or short stature homeobox-containing gene (SHOX) deficiency
· congenital malformations involving the pituitary (e.g., septo-optic dysplasia, posterior pituitary ectopia)
· chronic renal insufficiency
· some infants who are small for gestational age
· intracranial tumors in or near the sella turcica, especially craniopharyngioma
· damage to the pituitary from radiation therapy to the head (e.g. for leukemia or brain tumors), from surgery, from trauma, or from intracranial disease (e.g. hydrocephalus)
· autoimmune inflammation (hypophysitis)
· ischemic or hemorrhagic infarction from low blood pressure (Sheehan syndrome) or hemorrhage pituitary apoplexy
There are a variety of rare diseases which resemble GH deficiency, including the childhood growth failure, facial appearance, delayed bone age, and low IGF levels. However, GH testing elicits normal or high levels of GH in the blood, demonstrating that the problem is not due to a deficiency of GH but rather to a reduced sensitivity to its action. Insensitivity to GH is traditionally termed Laron dwarfism, but over the last 15 years many different types of GH resistance have been identified, primarily involving mutations of the GH binding protein or receptors.
As an adult ages, it is normal for the pituitary to produce diminishing amounts of GH and many other hormones, particularly the sex steroids. Physicians therefore distinguish between the natural reduction in GH levels which comes with age, and the much lower levels of “true” deficiency. Such deficiency almost always has an identifiable cause, with adult-onset GHD without a definable cause (“idiopathic GH deficiency”) extremely rare. GH does function in adulthood to maintain muscle and bone mass and strength, and has poorly-understood effects on cognition and mood.
Pediatric endocrinologists are the physicians who specialize in diagnosis and treatment of growth hormone deficiency and growth problems in children. Internist endocrinologists are the physicians with the most expertise in assessment and treatment of adult GH deficiency. Although GH can be readily measured in a blood sample, testing for GH deficiency is constrained by the fact that levels are nearly undetectable for most of the day. This makes simple measurement of GH in a single blood sample useless for detecting deficiency. Physicians therefore use a combination of indirect and direct criteria in assessing GHD, including:
· Auxologic criteria (defined by body measurements)
· Indirect hormonal criteria (IGF levels from a single blood sample)
· Direct hormonal criteria (measurement of GH in multiple blood samples to determine secretory patterns or responses to provocative testing), in particular:
· Subnormal frequency and amplitude of GH secretory peaks when sampled over several hours
· Subnormal GH secretion in response to at least two provocative stimuli
· Increased IGF1 levels after a few days of GH treatment
· Response to GH treatment
· Corroborative evidence of pituitary dysfunction
“Provocative tests” involve giving a dose of an agent that will normally provoke a pituitary to release a burst of growth hormone. An intravenous line is established, the agent is given, and small amounts of blood are drawn at 15 minute intervals over the next hour to determine if a rise of GH was provoked. Agents which have been used clinically to stimulate and assess GH secretion are arginine, levodopa, clonidine, epinephrine and propranolol, glucagon and insulin. An insulin tolerance test has been shown to be reproducible, age-independent, and able to distinguish between GHD and normal adults, and so is the test of choice.
Severe GH deficiency in childhood additionally has the following measurable characteristics:
· Proportional stature well below that expected for family heights, although this characteristic may not be present in the case of familial-linked GH deficiency
· Below-normal velocity of growth
· Delayed physical maturation
· Delayed bone age
· Low levels of IGF1, IGF2, IGF binding protein 3
· Increased growth velocity after a few months of GH treatment
In childhood and adulthood, the diagnosing doctor will look for these features accompanied by corroboratory evidence of hypopituitarism such as deficiency of other pituitary hormones, a structurally abnormal pituitary, or a history of damage to the pituitary. This would confirm the diagnosis; in the absence of pituitary pathology, further testing would be required.
Growth hormone treatment
GH deficiency is treated by replacing GH with daily injections under the skin or into muscle. Until 1985, growth hormone for treatment was obtained by extraction from human pituitary glands collected at autopsy. Since 1985, recombinant human growth hormone (rhGH) is a recombinant form of human GH produced by genetically engineered bacteria, manufactured by recombinant DNA technology. In both children and adults, costs of treatment in terms of money, effort, and the impact on day-to-day life, are substantial.
Treatment in childhood
GH treatment is not recommended for children who are not growing despite having normal levels of growth hormone, and in the UK it is not licensed for this use. Children requiring treatment usually receive daily injections of growth hormone. Most pediatric endocrinologists monitor growth and adjust dose every 3–6 months and many of these visits involve blood tests and x-rays. Treatment is usually extended as long as the child is growing, and lifelong continuation may be recommended for those most severely deficient. Nearly painless insulin syringes, pen injectors, or a needle-free delivery system reduce the discomfort. Injection sites include the biceps, thigh, buttocks, and stomach. Injection sites should be rotated daily to avoid lipoatrophy. Treatment is expensive, costing as much as $US 10,000 to 40,000 a year in the USA.
Treatment in adulthood
GH supplementation is not recommended medically for the physiologic age-related decline in GH/IGF secretion. It may be appropriate in diagnosed adult-onset deficiency, where a weekly dose approximately 25% of that given to children is given. Lower doses again are called for in the elderly to reduce the incidence of side effects and maintain age-dependent normal levels of IGF-I. In many countries, including the UK, the majority view among endocrinologists is that the failure of treatment to provide any demonstrable, measurable benefits in terms of outcomes means treatment is not recommended for all adults with severe GHD, and national guidelines in the UK as set out by NICE suggest three criteria which all need to be met for treatment to be indicated:
1. Severe GH deficiency, defined as a peak GH response of <9mU/litre during an insulin tolerance test
2. Perceived impairment of quality of life, as assessed by questionnaire
3. They are already treated for other pituitary hormone disorders
· Joint pain and muscle pain
· Fluid retention, and carpal tunnel syndrome
· Mild hypertension
· Visual problems
· Nausea and vomiting
· Antibody formation
· Reactions at the injection site
· Rarely, benign intracranial hypertension.
When treated with GH, a severely deficient child will begin to grow faster within months. In the first year of treatment, the rate of growth may increase from half as fast as other children are growing to twice as fast (e.g., from 1 inch a year to 4 inches, or 2.5 cm to 10). Growth typically slows in subsequent years, but usually remains above normal so that over several years a child who had fallen far behind in his height may grow into the normal height range. Excess adipose tissue may be reduced.
GH treatment can confer a number of measurable benefits to severely GH-deficient adults, such as enhanced energy and strength, and improved bone density. Muscle mass may increase at the expense of adipose tissue. Although adults with hypopituitarism have been shown to have a reduced life expectancy, and a cardiovascular mortality rate more than double controls, treatment has not been shown to improve mortality, although blood lipid levels do improve. Similarly, although measurements of bone density improve with treatment, no rates of fractures have not been shown to improve. Effects on quality of life are unproven, with a number of studies finding that adults with GHD had near-normal indicators of QoL at baseline (giving little scope for improvement), and many using outdated dosing strategies. However, it may be that those adults with poor QoL at the start of treatment do benefi].
Perhaps the most famous person who exemplified the appearance of untreated congenital growth hormone deficiency was Charles Sherwood Stratton (1838-1883), who was exhibited by P.T. Barnum as General Tom Thumb, and married Lavinia Warren. Pictures of the couple appear to show the typical adult features of untreated severe growth hormone deficiency. Despite the severe shortness, limbs and trunk are proportional.
One of the most known persons to have suffered from this deficiency is F.C. Barcelona striker Lionel Messi. He was diagnosed with the disease at the age of eleven and his family couldn’t afford its treatment, but that cost was covered by Barcelona in exchange for Messi joining the team’s youth departments. Like many other 19th century medical terms which lost precise meaning as they gained wider currency, “midget” as a term for someone with severe proportional shortness acquired pejorative connotations and is no longer used in a medical context.

Growth failure is a term used to describe a growth rate that is below the appropriate growth velocity (speed) for age. The term growth delay may refer to a situation in which a child is short but appears to be able to grow longer than children usually do, and thus, may not end up short as an adult. Dwarfism is a term that has often been used to describe extreme short stature; however, the term is unflattering and its use is often avoided. Growth hormone deficiency is sometimes called pituitary dwarfism. Short stature may be a normal expression of a person’s genetic potential and, therefore, the growth rate is normal. Short stature may also be a result of a condition that causes growth failure and a growth rate that is slower than normal. A child is considered to be short if he or she has a height below the 3rd or 5th percentile on a growth chart. About 3%-5% percent of all children are considered to be short. However, many of these children have normal growth velocities. The children who fall into this group include those with familial short stature or constitutional growth delay. Those with familial short stature are born with genes that determine their short height, and they usually have parents who are short. Constitutional growth delay is a term used to describe children who are small for their age but who have a normal growth rate. Of all children with short stature, only a few have a specific treatable medical condition. The most rapid phase of growth occurs in the mother’s uterus. After birth, the growth rate gradually declines over the first several years of life. At birth, the average length of a newborn is 20 inches; at 1 year, the average height is about 30 inches; at 2 years, the average height is about 35 inches; and at 3 years, the average height is about 38 inches. After 3 years and until puberty, linear growth continues at a relatively constant rate of 2 inches per year.
Growth Failure in Children Causes
Normal growth is the result of several factors, such as nutrition, genetics, and hormones. The following are possible causes of growth failure.
- Familial short stature: Children with this condition have parents with short stature. They have a normal growth velocity, and bone age is not delayed. They enter puberty at a normal time and often complete growth with a short adult height.
- Constitutional growth delay: Also called delayed puberty, children with constitutional growth delay have a normal birth weight, and their growth slows usually during the first year of life. From about age 3 to puberty, these children have an adequate growth velocity. Bone age is usually delayed, and puberty is late. Late puberty allows for more prepubertal growth, usually resulting in a normal adult height. Usually, children with constitutional growth delay do not show growth failure but a period of slow growth velocity occurs during the first year of life and just before puberty.
- Malnutrition: Worldwide, malnutrition is probably the most common cause of growth failure and is usually poverty-related. Nutritional deficiencies in developed countries are more often the result of self-restricted diets. Poor weight gain is often more noticeable than short stature.
- Diseases and disorders: Chronic diseases and systemic disorders that involve the nervous, circulatory, or gastrointestinal systems may be a cause of growth failure. Diseases or disorders involving the liver, kidneys, lungs, or connective tissue may also be a cause.
- Psychosocial dwarfism: This is a disorder of short stature or growth failure and/or delayed puberty. This often occurs in association with emotional deprivation and/or child abuse and neglect.
- Syndromes: Growth failure can be a feature of genetic syndromes, such as Turner syndrome and Down syndrome. It can also be a part of other syndromes, such as Noonan syndrome, Russell-Silver syndrome, and Prader-Willi syndrome.
- Endocrine (hormonal): Endocrine causes include thyroid hormone deficiency (hypothyroidism), growth hormone deficiency, or other hormone disorders. Thyroid hormone is necessary for normal growth; in children with hypothyroidism, growth is extremely slow. Children with growth hormone deficiency have normal body proportions, but they may appear younger than their actual age.
- Other: Growth failure may be related to intrauterine growth retardation (a condition in which children weigh less than 5 pounds at full term or who are small for gestational age if born preterm). Bone and cartilage disorders (called chondrodystrophies) may also be a cause of growth failure. Achondroplasia (one of the most common conditions that cause growth failure and short stature) is a genetic disorder of bone and cartilage. People with achondroplasia have a normal-sized trunk, short arms and short legs, and a slightly enlarged head with a prominent forehead. Adults with this condition are typically about 4 feet tall. There are other chondrodystrophies, such as hypochondroplasia, which are similar to achondroplasia but not as severe.
Symptoms of Growth Failure in Children
Symptoms of growth failure may include the following:
- The child’s height, weight, and head circumference do not progress normally according to standard growth charts.
- Physical skills, such as rolling over, sitting up, standing, or walking, are slow to develop.
- Social and mental skills are delayed.
- The development of secondary sexual characteristics (for example, men’s facial hair, women’s breasts) is delayed in adolescents.

The doctor or health-care practitioner will measure the child’s weight and height. He or she may also ask about the child’s birth weight and birth height. The doctor may take measurements of the limbs and trunk to determine the child’s body proportions.
The doctor may perform blood tests to check for hormones and to rule out specific syndromes associated with growth failure. The following blood tests may be performed:

- Serum electrolyte levels,
- Complete blood count and sedimentation rate,

The doctor or health-care practitioner may want to evaluate the child every three months while the cause of growth failure is being investigated. The doctor may refer the child to a pediatric endocrinologist (a doctor who specializes in studying hormones) for a more detailed evaluation of the possible causes of growth failure.
Routine well-baby checkups and periodic checkups for school-age and adolescent children is the best means of prevention.

The pituitary gland or hypothesis is a small gland about 1 centimeter in diameter or the size of a pea. Gonadotropic hormones react with receptor sites in the gonads, or ovaries and testes, to regulate the development, growth, and function of these organs. Prolactin hormone promotes the development of glandular tissue in the female breast during pregnancy and stimulates milk production after the birth of the infant.
Hormones of the Posterior Lobe (Neurohypophysis)
Antidiuretic hormone promotes the reabsorption of water by the kidney tubules, with the result that less water is lost as urine. This mechanism conserves water for the body. Insufficient amounts of antidiuretic hormone cause excessive water loss in the urine. Oxytocin causes contraction of the smooth muscle in the wall of the uterus. It also stimulates the ejection of milk from the lactating breast.
The pineal gland, also called pineal body or epiphysis cerebri, is a small cone-shaped structure that extends posteriorly from the third ventricle of the brain. The pineal gland consists of portions of neurons, neuroglial cells, and specialized secretory cells called pinealocytes. The pinealocytes synthesize the hormone melatonin and secrete it directly into the cerebrospinal fluid, which takes it into the blood. Melatonin affects reproductive development and daily physiologic cycles.
Dwarfism is a condition in which the growth of the individual is very slow or delayed. There are many forms of dwarfism. The word pituitary is in reference to the pituitary gland in the body. This gland regulates certain chemicals (hormones) in the body. Therefore, pituitary dwarfism is decreased bodily growth due to hormonal problems. The end result is a proportionate little person, because the height as well as the growth of all other structures of the individual are decreased.
Pituitary dwarfism is caused by problems arising in the pituitary gland. The pituitary gland is also called the hypophysis. The pituitary gland is divided into two halves: the anterior (front) and posterior (back) halves. The anterior half produces six hormones: growth hormone, adrenocorticotropin (corticotropin), thyroid stimulating homone (thyrotropin), prolactin, follicle stimulating hormone, and lutenizing hormone. The posterior pituitary gland only produces two hormones. It produces antidiuretic hormone (vasopressin) and oxytocin. Most forms of dwarfism are a result of decreased production of hormones from the anterior half of the pituitary gland. The most common form is due to decreases of growth hormone which will be discussed here. These decreases during childhood cause the individual’s arms, legs, and other structures to develop normal proportions for their bodies, but at a decreased rate. When all of the hormones of the anterior pituitary gland are not produced, this is called panhypopituitarism. Another type of dwarfism occurs when only the growth hormone is decreased. Dwarfism can also result from a lack of somatomedin C (also called insulin like growth factor, IGF-1) production. Somatomedin C is a hormone produced in the liver that increases bone growth when growth hormone is present. The African pygmy and the Levi-Lorain dwarfs lack the ability to produce somatomedin C in response to growth hormone. All causes of dwarfism lead to a proportionate little person.
Growth is the body’s response to different hormones. The forebrain contains a small organ called the hypothalamus, which is responsible for releasing hormones in response to the body’s needs for purposes of regulation. Growth hormone is produced in the anterior pituitary gland when growth hormone-releasing hormone (GHRH), is released by the hypothalamus. Growth hormone is then released and stimulates the liver to produce IGF-1. In return, IGF-1 stimulates the long bones to grow in length. Thus, growth can be slowed down or stopped if there is a problem making any of these hormones or if there is a problem with the cells receiving these hormones.
Causes & Symptoms
Pituitary dwarfism has been shown to run in families. New investigations are underway to determine the specific cause and location of the gene responsible for dwarfism. The human cell contains 46 chromosomes arranged in 23 pairs. Most of the genes in the two chromosomes of each pair are identical or almost identical with each other. However, with dwarfism, there appears to be disruption on different areas of chromosome 3 and 7. Some studies have isolated defects for the production of pituitary hormones to the short arm (the “p” end) of chromosome 3 at a specific location of 3p11. Other studies have found changes on the short arm of chromosome 7.
A child with a growth hormone deficiency is often small with an immature face and chubby body build. The child’s growth will slow down and not follow the normal growth curve patterns. In cases of tumor, most commonly craniopharyngioma (a tumor near the pituitary gland), children and adolescents may present with neurological symptoms such as headaches, vomiting, and problems with vision. The patient may also have symptoms of double vision. Symptoms such as truly bizarre and excessive drinking behaviors (polydipsia) and sleep disturbances may be common.
The primary symptom of pituitary dwarfism is lack of height. Therefore, a change in the individual’s growth habits will help lead to a diagnosis. Another diagnostic technique uses an x ray of the child’s hand to determine the child’s bone age by comparing this to the child’s actual chronological age. The bone age in affected children is usually two years or more behind the chronological age. This means that if a child is ten years old, his or her bones will look like they are those of an eight-year-old child. The levels of growth hormone and somatomedin C must also be measured with blood tests. Hypopituitarism may be gained or acquired following birth for several reasons. It could be due to trauma to the pituitary gland such as a fall or following surgery to the brain for removal of a tumor. It may also be due to the child’s environment (deprivational dwarfism). On examination by the doctor there may be optic nerve atrophy, if the dwarfism is due to a type of tumor. X rays of the area where the pituitary gland is located (sella turcica) or more advanced imaging such as magnetic resonance imaging (MRI) or computed tomography CT may show changes of the pituitary gland itself. Computed tomography, is an advanced form of x ray that will help determine the integrity of the bone and how much calcification the tumor is producing. Magnetic resonance imaging, will also help in the diagnosis. MRI is a type of imaging device that can visualize soft tissues such as muscle and fat. If the dwarfism is due to environmental and emotional problems, the individual may be hospitalization to monitor hormone levels. Following a few days of hospitalized, hormone levels may become normal due to avoidance of the original environment.

Take another deep breath and as you exhale roll your chin to your chest. On the next deep breath, exhale and roll your left ear to your left shoulder. Finally, take another deep breath and exhale as you roll your chin back to your chest. All of your movements should be steady and slow and you should inhale deeply. Repeat this set at least 3 more times. 3) As you perform the next exercise, called the Turkey Stretch, you will be able to feel the stretching and lengthening along the back of your neck. Because your neck is such an important factor in good posture, this exercise is great to increase height growth, and general fitness. Once again, you should keep your spine straight and feet flat while you sit in a chair. Act as if there were a string pulling up the crown of your head to make you as tall as possible. Focus your eyes on the area in front of your nose and raise your hand to your chin. Take a deep breath in, rest your hand on your chin and then begin to breathe out slowly. As you exhale, lightly push your chin into your neck until you feel the stretching on the back of your neck. When you finish exhaling stop and then repeat the set, you should do this at least 3 more times.

In the following descriptions in this chapter it is assumed that children are receiving good levels of nutrition. Restricted nutrition and sickness will both affect the way in which a child grows and develops. This should be taken into account by the coach when deciding the needs of the child. There are clear stages that children pass through from birth to adult. These stages are the same for boys and girls, but girls generally mature before boys. This is clearly shown in the diagram below.
Physical growth is obviously important to performance. We will start by looking at how the body changes during development. There are important changes in body size and proportions. These changes affect the way children can perform different skills and activities.
Patterns of Growth – Changes in Size
Children grow in size at a very fast rate. At birth infants are only about a quarter of their adult height. This final adult height is usually reached at about twenty years of age. There are four characteristic stages of growth from birth to adult:
- Rapid growth in infancy and early childhood
● Slow, steady growth in middle childhood
● Rapid growth during puberty
● Gradual slowing down of growth in adolescence until adult height is reached
Both sexes are of a comparable shape and size during infancy and childhood.
Patterns of Growth – Changes in Proportions
The physical proportions of the body at birth are very different from those of the adult. Some body parts grow more than others during development to reach the final adult proportions. The illustration shows the relative size of body parts at different ages.

Patience and encouragement from the coach during -this time will be of most benefit to the young woman. The sexual development which happens at puberty can bring physical difficulties for adolescent children, as well as causing them mental and emotional preoccupation. Coaches need to be particularly understanding with girls when menstruation begins. This may, but should not, inhibit their participation in physical activity. Menstruation is a normal biological process that commences with the sexual development at puberty. Male coaches in particular should understand what is happening to the bodies of the female athletes they coach. At puberty girls start to produce mature eggs in their ovaries. They will notice this because each month they will lose a small amount of blood through the vagina. This menstruation is also known as a period and generally lasts for about five days. This means that the right type of food needs to be consumed if we are to grow to our full height potential. As with most balanced diets, there are 6 food nutrient groups which we need to make sure is included in our diets if we are to grow taller naturally through nutrition. All of these play an individual part in both growth, and in keeping us generally healthy and illness free.
During this 7 part series we will address and talk about each of the above elements, and explain how they can be used to our advantage. We will start off at the beginning and talk about the importance of Minerals.
What’s Important About ?
An adequate supply of minerals is a crucial component of a healthy balanced diet. Minerals are used in just about every function our body undertakes from repairing itself to developing and fuelling our organs and muscles too. Minerals also play a very important role in making sure that our growth rate is at its optimum level, but to do this we need to consume the full range of minerals our bodies will need. Another secondary reason why we need to include minerals in our grow taller nutrition plan is because our bones contain a large percentage of minerals, so a lack of these could lead to compromised bone growth.
So What About Calcium?
Calcium is a very special mineral and it can be argued that in the case of grow taller diets, Calcium is possibly the most important mineral of all. It is actually a very abundant mineral found in many parts of your body from your bones to your hair and even your fingernails too! The only problem is that as humans, our bodies are unable to produce Calcium for ourselves, so we are required to consume the Calcium levels we need. Next It’s Phosphorous.
Phosphorus is another mineral that is found in huge abundance throughout the human body. This is probably because the vast majority of it is combined with calcium in your body to create the compound calcium phosphate, which is the source of your bones strength. It also has a secondary function which is just as important to your grow taller diet. Calcium phosphate carries very important information about cell regeneration as it forms part of your DNA. As it often works hand in hand with the calcium in your body, it is a sensible idea to make sure you also have a high intake of phosphorus rich foods in your diet. Fortunately for us, this time phosphorous is quite easy to include in our daily diets as it is found in a wide range of produce such as dairy produce and meat, fish and even in vegetables too. However, phosphorus does have a negative side to it too. It can in large quantities interfere with your bodies abilities to use calcium. Vitamins are basically organic substances which are essential for our growth and development at every stage during our life. It is therefore really important that we make sure we are consuming an adequate level of vitamins throughout our daily lives. 2) Water-soluble vitamins – Again, these vitamins are also found in meats and fish but they are also found in most vegetables and fruits. Unlike fat soluble minerals they are transported in water instead of fats which means it is not possible for your body to store these vitamins as your system will naturally flush them through in your waste products like urine.
How Can I Get All These Vitamins From My Diet?
It is not too difficult to achieve an adequate level of vitamins in your daily diets. Most of the foods we know to be good for us contain high levels of vitamins, especially those foods such as green vegetables and fruits. Now you’ve considered the other elements of a diet that you need to maximize your efforts to grow taller, you need to think about fats.
Most of probably think of fats as a very negative element in our daily diets and most likely think we should avoid these at all costs. However that is not necessarily the case.
There are in fact two types of fats which we as Humans consume as part of our daily eating habits and you should be aware of where fats fit in to your ‘grow taller diet’ plan.
The two types of fats we consume are known as saturated and unsaturated fats. A diet which is excessively high in fats will of course stunt your growth efforts, so let’s talk about the two different types of fats:
The saturated fats are those which we consider to be the most harmful to us as people. They contain extremely high levels of cholesterol which can block your arteries and cause fat deposits to be left around your arteries and organs which can then sometimes lead to illnesses such as heart disease.
Saturated fats can also contain lot of calories which tends to cause you to put on weight at a faster rate than other foods.
As we have already discussed, putting on weight will counter your efforts to grow taller as it will make your body appear to be wider and therefore shorter too.
Primarily saturated fats come from animal products like red meat and fatty dairy items such as whole milk and eggs so try to control the amount of these foods you eat.
The second type of fats we will discuss are known as ‘unsaturated’ fats.
These types of fats are less harmful to you as they contain a far lower calorific value and also contain lower levels of cholesterol too. As a Human, you do require a certain level of fats to keep yourself healthy and for certain functions to be carried out effectively in your body.
Due to this purpose you should try and make sure you eat more unsaturated fats than saturated in your grow taller diet plan.
There is now a wide range of products available which contain unsaturated fats as opposed to the more harmful saturated.

These fluids can be consumed as one of the following: plain water from a safe drinking source, juice from fresh squeezed fruits or bottled juices, warm drinks such as tea or coffee but beware of taking in too much caffeine as it could lead to headaches and restlessness.
Alcoholic drinks do not qualify as fluid intake as the diuretic effect they have actually means that nutrients are washed from your body and you will lose fluids too.
You may be wondering about what else to eat to grow taller. Well you may also be surprised to find out that a very common mistake lots of people make when trying to get taller is that they include far too much carbohydrate in their diet.
